Submitting your biometrics

To submit your biometric results, please ensure the following information is included:

• Your biometric results
• Your name
• Your date of birth
• Your employee ID (or your spouse’s employee ID, if applicable)

Once you have this information, you can submit it via email to [e-mail], or fax it to Hy-Vee Benefits at 515-559-2598.
