Activity/Exam not marked as complete on portal

If you do not see your exam marked as complete, please note that Hy-Vee cannot guarantee receipt of all completion files from the vendor.

We recommend uploading proof directly to the wellness portal.

When uploading your exam documentation:

• The file name should not include any periods, as this can cause upload errors.

• The document must include:

  • Your full name

  • Employee ID

  • Type of exam

  • Confirmation that it was a preventive exam

  • Physician’s name

Uploading your documentation ensures your exam is recorded correctly and marked as complete.
