We recommend uploading proof directly to the wellness portal.
When uploading your exam documentation:
The file name should not include any periods, as this can cause upload errors.
The document must include:
Your full name
Employee ID
Type of exam
Confirmation that it was a preventive exam
Physician’s name
Uploading your documentation ensures your exam is recorded correctly and marked as complete.
